This is a mandatory part of the Academic Probation
Program for students deemed eligible for the program.

Click the button below to complete the Academic Self-Assessment & Personal Reflection. It is mandatory that the questionnaire be completed and submitted by MAY 31, 2019 to allow time to review your case. A recovery plan meeting will not take place unless the questionnaire is submitted.
Call to schedule a recovery plan meeting by JUNE 7, 2019: Call 888-2910 between 8:30 a.m. and 2:30 p.m. to set up a meeting.
(If you live more than 60 minutes away from the college, you may schedule a phone meeting. Please follow the same instructions as for a meeting on campus.) This mandatory meeting must take place by JUNE 28, 2019.Note that the college is closed for Memorial Day on Monday, May 27, 2019. Please do not leave messages on voice mail requesting an appointment.
